National Instruments DAQPad-6020E Multifunction DAQ  

The DAQPad-6020E (Part Numbers: 777474-01, 777474-04, 777474-06, 777474-07, 777704-01, 777704-04, 777704-06, 777704-07, 777703-01, 777703-04, 777703-06, and 777703-07) is a legacy Multifunction DAQ by National Instruments. This device has multiple part numbers because it has different variations with mass termination or BNC connectors. These versions are a 30 cm enclosure, but there is also a version which is a 15 cm enclosure. Several power options are also available for each variation.
